IT - Lost in Derry 4 Mod feels like you’re walking into a scripted horror map that just happens to be inside a normal world. You’ll run into new characters and trigger cutscenes by interacting with them using specific items, which can kick off spooky encounters and surprise spawns while you’re out exploring. Nights get extra tense with things like Watcher Pennywise silently appearing, and random red balloon moments that keep you checking your surroundings.

The mod pushes you to hunt structures, especially the Hospital (found in plains) and the Sewerage areas, plus special locations tied to events like the cursed music box. Fortune cookies add a nasty twist too: you eat one, get a prediction, and then you might have to deal with whatever shows up right after. There’s also Fear as a new resource, letting you craft a Fear Block that screams when placed or broken, which is perfect for creepy bases or trap builds.



Boss fights are the main flex. IT - Lost in Derry 4 Mod includes a Pennywise 2017 boss that forces nightfall and runs through multiple evolving forms, and a Pennywise 1989 boss that lives in the sewer with its own set of transformations. If you win, you can score a rare Hand artifact, and keeping it in your first hotbar slot boosts your melee damage by 10.
